Requirements

With a Physical ATM Card

• One (1) Photocopy of ATM Card (FRONT ONLY), no need to write your Account Number as long as the “SA” part is readable.

• Authorization Form

• One (1) Photocopy of Student ID and Government-accepted ID with three (3) specimen signatures. (If no valid ID, you may submit your Student ID only)

With a Digital ATM Card

• One (1) Printout of the Digital ATM Card. Write your “10-digit Account Number” below the Digital ATM Card (see visual guidelines below).

• Authorization Form

• One (1) Photocopy of Student ID and Government-accepted ID with three (3) specimen signatures.

Guidelines and Processes

With a Physical ATM Card

• One (1) Photocopy of ATM Card (FRONT ONLY), do not include the back part of the card as it exposes the CVV. (No need to write your Account Number as long as the “SA” part is readable.)

• Authorization Form (you may obtain a copy at the Scholarship Office)

• One (1) Photocopy of Student ID and Government-accepted ID with three (3) specimen signatures. (If no valid ID, you may submit your Student ID only)

With a Digital ATM Card

• One (1) Printout of the Digital ATM Card (if you do not know where to locate the “10-digit Account Number,” you may see it once you open the app.) Blur or erase the CVV part. If you already submitted your virtual card with the CVV included, the Office will erase it.

• Write “10-digit Account Number below the Digital ATM Card. Do not write the “Card Number” since it is not the “Account Number.”

• Authorization Form (you may obtain a copy at the Scholarship Office)

• One (1) Photocopy of Student ID and Government-accepted ID with three (3) specimen signatures.

Other Guidelines

• Only those students who are opening an account will request an Endorsement Letter. If you already have a Piso Plus Account and are already accessing the app and Virtual Card, no need to request Endorsement.

• Please obtain a copy of the Authorization Form at the Scholarship Affairs Unit located at 2nd Floor of Student Affairs and Services in front of the Old CAS Building.

• Double-check your “10-digit Account Number” before submitting at the office.

• For Authorization filling, you may start at Surname, Given Name, and Middle Name, not Middle Initial.

• Academic Year / Term Covered will be 2025-2026 2nd Semester

• Do not forget to check the back part of the Authorization Form.
